Investigators noted that the brakeman of one of the carriages activated the brakes, but they were not sufficient to avert the calamity.
Both carriages had moved only six meters after departing their stations on Wednesday, around 6 p.m., when the connecting cable broke, according to the report.
Sixteen people lost their lives in Wednesday’s crash, including 11 foreigners: five Portuguese, two South Koreans, one Swiss, three Brits, two Canadians, one Ukrainian, one American, and one French.
Lisbon is set to hold local elections next month, with current mayor Carlos Moedas, a former European Commissioner, aiming for a second term.
